can you diagnose this.... 2.7L EFI

2002 model 2.7L EFI petrol 3RZ-FE engine 4X4
Manual, 230,000klm

Getting a weird sort of shudder, like lack of fuel, hesitation, bunny hop, chug kinda situation....
Drives on highway fine.
Idles perfectly, however sometimes seems to be a bit low at around 600RPM?.... only sometimes, usually sits just under 1000RPM.
It only happens on acceleration when the revs are between around 1500 - 2000 RPM, once the revs are over 2000 rpm it clears and runs fine, this is however an intermittent occurrence.
Clean oil, new filters etc, have cleaned MAF....
Sometimes when Im sitting at the lights, and go to take of, the initial rev before engaging clutch seems to have a bit of hesitation like there is a backlog of fuel, on acceleration from 1st to 2nd sometimes chugs and splutters then once its past that 2000rpm mark and shifted into 2nd runs fine (once again this doesnt happen all the time)
any ideas?
does it just need a tune?
injector clean?

dont wanna spend much as selling vehicle soon....

Note, last time I serviced it, oils filters etc whilst I was under the hood I noticed a bit of slack in the accelerator cable and re tensioned it so there was next to no slack, above problem seemed to arise after that sometime however I didnt notice it straight away to be related to what I had adjusted....anyways I put some slack back into the cable but problem still occurs..... :|

Turned out to be a misfire, threw some new plugs and leads on and it fixed the problem. Gotta love simple inexpensive fixes! Might get better fuel economy too now that shes running on all 4 cylinders again ha!
