vacuum system diagram

I keep getting an EGR P0405 error 20 seconds after the engine is running. I've checked all connections, the lift sensor have the correct ohms as per the factory service manual. The only thing I can think of is that maybe the vacuum lines somewhere got crossed and it's causing the issue. Can anyone point me to a vacuum system diagram? Or take pics of their system to compare? The engine is out of a 2009 Hilux w/ the 1KD-FTV.

Thanks for the response. I've checked everything and it appears correct. I've measured the ohms on the lift sensor and it checked out perfectly. I've checked the wiring harness connections, and wiring between the ECU and the EGR sensor plugs as per the FSM. Scratching my head on this one....

that's my next step is to verify that the outlet on the manifold is actually flowing. I've put a vacuum on all feed lines and it holds pressure so no leak. Thanks for the help so far. I've just ordered a Intelligent Tester II from the UK so I'll post up the results as they come in.
