Hey Craig. I had a similar issue with mine. I changed the fuel filter and when I removed it I blew from the clean side back and found out it was full of crap. Like black slimy diesel death crap. Changed the filter and it helped but wasn't the saving grace. I checked my fuel trims and found that they were defaulting to 0% instead of the normal graph. Long story short, my fuel pump wasn't keeping up. Changed it and I haven't had a drama since.
If you're going to go down that road though. Go to Toyota, get a price (should be about $999 excluding labour or $660 if you ask for trade discount), tell them to eat a fat one, go to super cheap auto and get a price (should be about $380 for a genuine denso pump. I paid $120 because I knew someone who worked for the super cheap group) and then decided if you really want to replace it as it is a massive pain in the ass and I mean monumental. That's coming from a mechanic with almost a decades experience.
The other issue you may be having is the ACIS? AICS? I forget which it is but it's the acoustic induction control system. It uses long runners for low rpm to increase torque and opens a valve to shorten the runners to increase power. This happens just after 2000 rpm and that increase in air calls for more fuel which maxed out the pump on mine.
You may be having issues with yours sticking which could cause a similar issue. I coul talk forever about this so let me know if you need more info or send me pm and I'll give you a call.
